Please sign the following waiver to participate
Please complete your waiver prior to visiting Titletown to participate in Titletown Dodgeball Tournament:
- Each adult (18 and older) must complete his/her own waiver.
- You will receive an email - upon successful completion of the online waiver. Please keep this accessible for check-in.
