Please complete your waiver prior to visiting Titletown for Fitness Activities programs. Please note:

• Each adult (18 and older) must complete his/her own waiver.
• Each adult may add up to 10 minors per waiver.
• You will receive a confirmation email upon successful completion of the online waiver.

• Each adult (18 and older) must complete his/her own waiver and may add up to 10 minors per waiver.
• You will receive an email upon successful completion of the online waiver. Please keep this accessible for check-in.
• Your completed waiver (and associated QR code) is valid for the day only.
